Forms for Participation

These are posted on the TCS website.  From home page, put mouse on Athletics and then select SIGN UP FORMS.  In addition, links to the three forms will be found below.

The three forms needed are:

Track Permission Form Click Here
Sports Medical Form:  Click Here
Parent/Athlete Agreement Form:  Click Here

The latter two are good for one school year so if handed in for other sports do not need to be completed again.
